IF YOU ARE A PATIENT, please sign in using the user-id and password you were assigned.



HERES WHAT TO DO:

1) Login to the Patient Login section and you will be taken to the HOME screen where you

will see a MY TO DO LIST.

2) Please click on each link under the heading MESSAGE and then read and complete each

form we have initially placed in your MY TO DO LIST.

Click Documents. Click Create New Documents Click Select Document Form Click on a document, then Create Document

Please add your "signature"  using your password, and fill out all documents available to you. Your clinician will see them immediately. There are **several** forms. Please fill out ALL forms including a Release of Information to your primary care doctor. Some will say "pending" because your clinician needs to sign them as well.

Payments are due in full at each session. If payment is not received in full, your clinician will be unable to provide further appointments as your account will become disabled. We now require two full business days notice for any cancellations.

Did you forget? Your next appointment time and date is available to you anytime using your login ID and password.



PASSWORD PROTECTION INFO: The information you submit is secured and no one else has access to your clinical information as it is password protected. Once logged in, you will have the option of changing your password by clicking on change password on the left top portion of the screen. In order to get back to the main page to continue with the To Do List, click the back button at the bottom of each form.



FOR PATIENTS UNDER 18: a parent or guardian will need to fill out each form for the client and electronically sign each form.
